Returning home from hospital or rehab can be overwhelming and it may be difficult or impossible to leave your home to see a healthcare provider. We will come to your home, review all available hospital/rehab records, do a full assessment, review current symptoms, reconcile medications, provide prescriptions if necessary, and assist with coordination of home health referrals or specialist referrals, if needed.
