I have always enjoyed providing primary care to people in their homes and now more than ever the need is here.

I provide care to people needing chronic disease management for such conditions as:

• Diabetes

• Osteoporosis

• Chronic kidney disease

• Chronic heart disease

• Arthritis

• Headaches

I provide transitional care to people just discharged from hospital or rehab.

This includes:

• Reconciliation of your medication list.

• Ensuring that you have prescriptions for medications.

• Referral for home health if needed.

• Referral for outpatient therapy if needed.

• Assessment to ensure that you are responding to new treatments or medications.

• Review follow-up appointments with specialists.
